Divorce Intake

What your attorney needs before your first appointment

Home › Memphis Divorce › Divorce Intake

Fill this out before your appointment. It takes about fifteen minutes and it saves an hour at the office. Anything you do not know, leave blank and bring it with you.

Leave Social Security numbers off this form. Yours, your spouse's, and the children's all get collected in person when you come in. Nothing here needs them.

About Your Spouse

Fill in what you know. Blanks here are normal and nothing stalls because of them.

Your Children

Minor children only. One entry for each.

The children's portion of the premium, not the whole family plan
Copays, medicine, anything insurance does not cover
Alternate years, split them, one parent claims all, or explain
If there is an order
Mother, or whoever you list first
Enter both numbers. They have to add up to 365.
Real Estate

Houses, land, rentals, a vacation place, an empty lot. One entry for each.

Everything Else

Cars, furniture, bank and investment accounts, retirement accounts, anything worth listing.

Anything Else

Only your name and phone number are required. Everything else can stay blank, and you can bring what you could not find to your appointment. Sending this form does not create an attorney and client relationship, and nobody is your lawyer until both sides agree to it in writing.

Got it. Your intake is in. Somebody will call you to set the appointment.

Stuck on Something?

Call before you send it. Blank fields are fine, and nothing on this form has to be perfect.
